This European project aims to explore the impact of digital technologies on the patient-physician relationship in the context of chronic care. We will design an innovative tool that maps the patient journey, identify key pain points and assesses the role digital tools may have played at each stage.

The ultimate goal is to provide European health authorities with a practical, evidence-based tool to help them better implement digital technologies within their national care pathways and ensure they serve the patients who need them most.
